I'm not sure if this is a bug or not, either way I think it's something that needs a fix.

Limb dismemberment is brutal and awesome, when it's fair.
By that I mean for example if you get struck and your arm is sent flying off; that's fair.
What is not fair is when you're lying down unconscious and your limb's condition is deteriorating, so as you lie there and you hit the -100 mark, your limb just "falls off". That is not fair; that is not fun. Especially when you have your medic patching them up, but since the limbs are last in the priority order, by the time you've stopped the bleeding, patched head, chest, etc. Your limb has very likely reached -100 and drops off.

On a side-note: I wouldn't mind in the future if heads started falling (read: flying) off as well. ;)

Come on. Have you not heard of necrosis, gangrene? I'm having a squad of 25, near current limit, and i got 1 medic per 10 people, so it's 3, to round up, and i've not had a single case where medics didn't managed to patch everyone in time. Yet again, i'm not jumping into battles i obviously can't win without severe losses.

Nevertheless, I think you can lower the dismemberment severity in the options if you'd like. But i find default value completely reasonable. Plus, you've actually got a little time inbetween -100 and fall off.

P.S. Oh, you find slowly deteriorating wound leading to dismemberment unfair, but insta-decapitation is fine by you?

Unless, you know, you're a low level team of 7 running through Holy Lands and a pack of wolves spots you and attacks you that you can't defeat. I'm sure glad that your team of 25 members with 3 medics is relevant in this case.
Or when you just start out the game and a squad of hungry bandits decide that they want your food (that you don't even have) so the come and beat you up. Sure glad you never were in that position.

My point is that statement was completely unnecessary and irrelevant.

Or the fact that in loading screen tips it actually states that "if your limb gets hit by a sharp object while it's -100 it will be dismembered" (or something along those lines), implying that it has to be struck to fall off, not slowly deteriorate to fall off, which is more logical.
Necrosis doesn't cause your limbs to detach from your body.

All I'm saying is:
Hit weak limb -> dismembered.
Weak limb in bad condition and worsening -> can be fixed.

Who said "insta-decapitation"?
Obvious implication that -100 head would lead to decapitation, since -100 head means that you are already dead.
This could only be achieved by someone hitting real hard while the target is still standing, since if the target is already unconscious you wouldn't be able to hit them, so it would be a fitting brutality to achieve.

You've got a point, i completely forgot about low-level situations.

You've mentioned a medic yourself though, and i was merely trying to say that if medics are sufficient, they'll make it in time, and if they aren't, then you should probably consider some changings to your health program.

Either way, as a rule of thumb for Kenshi, if you see something that'll probably rip you into shreds - you run like hell or you avoid it. If you're careless enough to stumble across a pack of dogs, or to not run from a group of bandits, well, there'll be consequences.

Does it also says in a loading screen that a hit to -100 dmg is the only way to loose a limb?

Necrosis is exactly what causes limbs to be detached from the body. By a surgical instruments, of course, but they're still gone forever.

I'm not quite sure what we are getting excited about here, since there's an option that allows to lower the dismemberment severity to exactly the point you're proposing: [rare] it's only lost when -100 dmg is achieved with a single strike.

Change limb dismemberment to 'rare' in the 'general' section of the options.
M'seductress.
