Mouse Stutter and Frame Stutter - FPS ISSUES

I have been experiencing frame and mouse stutters during my gameplay. I have tried multiple mods and Nvidia optimizations with no avail. When I am not moving the camera and characters are running around or just viewing the city the frame rate runs smooth, 110+ fps but will stutter every 4 seconds or so. When I middle-mouse to rotate the camera as well as adjust the height of the camera the game stutters heavily.

Moving the mouse in-game causes it to skip across the screen, nothing close to the smoothness of a hardware cursor outside of the game. On the same note, I have tried using the hardware cursor option in-game to avoid the mouse stutter issue, but the frame stutter persists as well as when I middle-mouse rotate, the mouse will be anchored at the position I pressed middle-mouse, and rubberband back to that position when I rotate the camera.

For specs:
GTX 2080
32 GB Ram
Installed on SSD
CPU: i9-9900kf 3.60GHz

Again I have tried almost every optimization mod and nvidia optimization that I could find. I also have tried changing all of my settings as low as they could go, v-sync off/on, fullscreen/borderless, all my mods on/off and a fresh install.

If anyone has any tips or advice on the issue, anything would be appreciated.
