I'd think having this on the player's starting (primary) character would be sufficient. I tend to play my games that way too - no matter how many troops I gather to follow me, there is always a leader. That guy should have an individual rep with other factions (and NPCs) to reflect their impact on the world.

Once a king captured about 200 soldiers. He told his soldiers to rip both eballs off 199, and only one of 1. He then made that soldier guide HIS soldiers with only one eyeball back to his country. Imagine THAT for your evil-spreading tactics.

What would make combat more interesting would be to add an option to change the target of your attacks E.G. an option to go for legs or Arms instead of the body.

If you are fighting an opponent with much greater skill than you, breaking through an opponents defense is more difficult to get to their center of body. I took German long sword for 2 years (and i know this is eastern weapon based, but the basic principles still apply) The point was to keep the geometry of your sword in such a way that it left no openings or created false openings in order to entice an attack.

anyway, fighting a much more skilled opponent means you are not going to easily break through their attacks to get to their vital body parts and are essentially forced to aim for limbs instead, hampering their combat ability in order to get to those points. maybe i just want to run away and feel like taking some swipes at the legs and hope i get lucky and break something. Perhaps I'm actually trying to be David and kill a Goliath, hoping to put my opponents arms out of commission so that i may capture or finally defeat him, and loot his body.

Either way, hits on limbs are more common than body hits as putting your foot too far forward in your combat stance just a little, can leave you open to get hit without being able to reach your opponent EVEN if your weapons are the same length.

Implementing it would have to be on a less important scale. the current combat style would exist, the option to target particular body parts would be an extra option. selecting it would slow down your attacks considerably as you are focusing on certain parts of a body and being less opportunistic, the enemies melee defense skill would negate the hit chance, yet being able to target body parts would stop you semi-randomly hitting parts of an enemy and spreading the damage over all parts.

on the other hand if we assume you are a highly skilled swordsman fighting rookies you should easily be able to break through their defense and target vital parts of the body like the stomach, chest, or head. finishing them off without need for hampering their combat ability through hurting limbs. as their sword work would be so poor it leaves openings.

continuing onwards, parrying, this would be an amazing addition allowing the player to parry an attack and perhaps damage an enemies limb, focusing on parrying would reduce your attack frequency drastically but would pay off in your defense perhaps if maybe your defense skill is much higher than your attack.

I think this would be a great addition to the revamped medical system. Again stressing i do not want to have the current semi-random body hitting gone, i just would like to see targeting implemented at least partially as i think this game would be even more amazing with it.

Completelly agree with sparkz, i would easily see it as a tab of the char stats/inventory, with a body pic, and rates of hits on each bodypart, then you could have complex teams, like two agile girls using a light katana with 40% of hits on each arm, 10% on each leg, and a plated warrior with a 97Kg heavy club that have 100% hits focused on the head. quickly the enemy would find himself with it's two arms broken, and be finished by a violent blow on his head without being able to dodge it.

Another possibility is to allow programed combo. eg: You got your warrior, he's fighting in random mode and you notice that he's a bit slower than your enemies. You could choose to program the first hits on an enemy, like 2 hits on the right leg, 3 on the stomach and 3 to the head, hoping that your enemy won't dodge the hits on it's legs and runaway.
