Need Help Understanding TOWN OKAY

Town = Malnourished?
Is it possible to create a world state that parses for, in some manner and fashion, the malnourishing of and potentially even degree to which a town is malnourished? Maybe something like a "town is not malnourished", "town is partially malnourished" and "town is fully malnourished".
