The outpost info window (which appears above the left most info panel with name of outpost, research and power info etc) will usually appear upon creation of an outpost, by building something. It will also indicate that the settlement is created.
However, if the first thing being built is a wall (used makeshift, not sure about defense), the settlement becomes bugged and could not detect that the player's faction outpost is created. This is evidenced by the outpost info part (outpost name, current research, power info) not showing at all.
Other visible outcome include all the buildings that the player build get assigned to NPC town nearby, instead of belonging to player outpost. The buildings themselves seems to be usable otherwise. (though trader visit, base raids etc are probably bugged as well as a result). Not totally confirmed, but power production and use seems to be from the NPC town that the building is assigned.
I have not discovered any solution or workaround once this bug occured. Even if I build other buildings (non-walls) after the walls, it does not trigger the proper player outpost creation. I did an experiment by importing a new game, then build a generator first, which triggers outpost creation properly. But when I created a wall, the bug occured again, and even when I build a generator after the wall, the outpost info window did not appear.
Replicate:
Start/Import into new game (preferably with makeshift wall research)
Build makeshift wall as first building in base. (bug starts here)
Workaround:
Do not start an outpost with walls. Start with something else to trigger the outpost creation first.

Below is a processed screen shot which shows the info screens of two gates side by side. Coincidentally, in the game where I discovered the bug, each of them belong to a different town. The town name is highlighted with the red lines.
